Financial Policy Statement

Patient is ultimately financially responsible for all charges regardless of insurance involvement.
Patient is responsible to pay any deductible, co-insurance, copay, or any services deemed as a "non-covered benefit" by his/her insurance carrier at the time of services are rendered.

It is patient responsibility to verify benefits from his/her insurance plan and to make sure all proper authorizations/referrals have been obtained.

It is patient responsibility to tell Apex representative in advance if his/her insurance company requires pre-authorizations of procedures.

It is patient responsibility to keep Apex informed of insurance/address changes.

It is patient responsibility to pay outstanding balances within 30 days of notification. Failure to pay outstanding balance within 30 days of notification will result in termination of care from Apex.

Prior to any treatment being rendered, patient must agree and sign Apex Behavioral Health Dearborn PLLC Financial Policy Statement.

If patient does not want to sign this policy in advance of first visit, the services will not be provided and it would be considered as non establishment of patient - therapist relationship.
